School fees:            $45.00  Registration

                                $35.00  Milk per semester ($70.00/per year)

                                Physicals & Immunization Records

                                Proof of Residency

All kindergarten parents MUST submit the state required physical form, immunization record, a certified copy of the birth certificate, and proof of residency to register their child for school. You will be asked to provide these items before proceeding through the registration process. 

Birth Certificates:

If your child was born in Champaign County, and you do not have a certified copy of their birth certificate, you can purchase a copy at the Brookens Building, 1776 East Washington, Urbana.

Former PreK students:

Your child must have a new physical for kindergarten.  We already have a copy of the birth certificate on file.

Proof of Residency:

Each school year, parents of all Pre-K and Kindergarten students are required to provide proof of residency within the district at registration, by providing the required documentation listed below.  Please be sure address on proof of residency matches the address on your child’s registration form.

Please bring one of the following items for proof of residency:

• Property Tax Bill
• Rental Agreement/Lease
• House Closing Documents
• Mortgage Payment Receipt. 

Or, present two of the following items for proof of residency: 

• Driver’s license
• Vehicle registration
• Voter registration
• Current public aid card
• Most recent gas, electric or water bill
